Please report any incidents involving Derby RC members or equipment, or navigational concerns about other crews on the river, through this form within 24 hours of the incident.

Examples of incidents to be reported are:

- near miss with the bank or another river user   
- hitting the bank
- hitting a bridge with boat or blades

- hitting debris 
- damages to boats or blades
- damages to land training equipment

- capsize   

Accidents happen, so this form is not for allocating blame, it's to ensure that the equipment is checked over appropriately before next use and to assess our frequent incidents so that we can plan training and awareness moving forward.
